Endoscopic diagnosis of secondary aortoesophageal fistula.

چکیده
is a catastrophic complication of endo− vascular graft placement [1]. The typical symptom of secondary AEF is massive gastrointestinal bleeding with a history of thoracic aortic aneurysm repair [2]. En− doscopy is the most sensitive and specific diagnostic study [3]. Endoscopy should be carefully performed, as it excludes other, more common causes of upper gas− trointestinal bleeding, but should be ter− minated if a fistula is identified. We pres− ent an endoscopic finding of secondary AEF. A 60−year−old woman was diagnosed with a mycotic thoracoabdominal aortic aneurysm and underwent resection of the aneurysm with an in−situ prosthetic interposition graft. Two weeks later, she developed massive hematemesis with hypotension. Emergency esophagoscopy revealed that the graft had eroded into the upper esophagus, with active bleed− ing (l" Fig. 1). Angiography with endo− vascular stenting and coil embolization were performed but failed to control the bleeding (l" Fig. 2). The patient died from exsanguinating hemorrhage.

A Cause of Mortal Massive Upper Gastrointestinal Bleeding: Aortoesophageal Fistula
INTRODUCTION Aortoesophageal fistula is an uncommon but mortal cause of massive upper gastrointestinal bleeding. The most common causes are thoracic aortic aneurisym, foreign body reaction, malignancy and postoperative complication. It can be seen in different pattern on upper gastrointestinal endoscopy. There are surgical, endoscopic and interventional radiological treatment options, however, ...
متن کاملSecondary Aortoesophageal Fistula Associated With Aneurysmal Graft Infection by Coxiella burnetii.
Aortoesophageal fistula is a rare and serious condition that carries a high mortality rate. We present a case of overt gastrointestinal bleeding from an aortoesophageal fistula in a patient with chronic infection of an endovascular prosthesis with Coxiella burnetii.
متن کاملA Case of Secondary Aortoesophageal Fistula Inserted a Covered Self-Expanding Esophageal Stent to Control Gastrointestinal Bleeding
A 73-year-old man presented with melena. After a thorough workup including esophageal endoscopy, computed tomography scans, and esophagography, the diagnosis of secondary aortoesophageal fistula was made. Two years previously, he had undergone endovascular stent-graft repair for the dissection of his descending thoracic aorta. Because of the generally poor condition of the patient and the high ...
متن کاملAortoesophageal fistula associated with tuberculous mediastinitis, mimicking esophageal Dieulafoy's disease.
Aortoesophageal fistula is a rare and lethal disorder that may result from primary diseases of aorta or esophagus, aortic bypass graft, ingestion of foreign body, trauma, surgical procedure or instrumentation. Tuberculous fistula is extremely rare. We present a 27-yr-old female patient with aortoesophageal fistula associated with tuberculous mediastinitis. The patient experienced massive hemate...
متن کاملAortoesophageal fistula as a complication of thoracic aorta aneurism stent grafting – a case report and literature review
BACKGROUND Endovascular stent grafting is performed in patients with aneurysms of aorta or other major vessels. The procedure is considered to be generally safe, with a low risk of complications, the most common of which include endoleaks, stenosis or thrombosis at the stagraft and itsmigration. Very rare complications include aortoesophageal and aortobronchial fistulas (0.5-1.7% cases). CASE...
